MEMBERSHIP FREEZE AND CANCELLATION POLICY
MEMBERSHIP FREEZE
By requesting a freeze of my Image Sun Tanning Center membership, I acknowledge and agree that my membership will not immediately stop billing at the regular rate.
My next regularly scheduled membership payment on the 25th of the month will be processed as my final regular membership before the freeze begins. I will continue to have access to my membership benefits through the 24th day of the following month, after which my membership will be placed on freeze.
While my membership is frozen, I authorize Image Sun Tanning Center to charge a $5.00 monthly freeze fee on the 25th of each month. A membership may remain frozen for a maximum of 12 months.
All monthly freeze fees are non-refundable and have no cash value.
I understand and agree that all membership freeze fees are nonrefundable and will not be credited toward future membership dues, services, products, or other purchases, regardless of whether I later reactivate or cancel my membership. No cash refund, account credit, or other reimbursement will be provided for any freeze fees paid, except as required by applicable law.
MEMBERSHIP CANCELLATION
I acknowledge that my Image Sun Tanning Center membership requires a minimum of thirty-one (31) days' advance notice to cancel. Membership payments are processed on the 25th day of each month. Because of the 31-day notice requirement, one final regular membership payment will be due after my cancellation request is submitted.
For clarity, if my cancellation request is received on or before the 24th day of the month, my regularly scheduled payment on the upcoming 25th will be my final membership payment.
If my cancellation request is received on or after the 25th day of the month, my final membership payment will occur on the 25th day of the following month.
Following the final membership payment, I may continue to use my membership through the 24th day of the following month. My membership will terminate at the end of that paid membership period, and no additional recurring membership dues will be charged after the effective termination date.
Submitting this cancellation request does not immediately terminate my membership, stop payment required during the 31-day notice period, or eliminate any amounts already due under my membership agreement.
MEMBER ACKNOWLEDGMENT
BY SIGNING BELOW, I ACKNOWLEDGE THAT I HAVE READ, UNDERSTAND, AND AGREE TO THE MEMBERSHIP FREEZE, AND CANCELLATION POLICY ABOVE. I SPECIFICALLY ACKNOWLEDGE AND AGREE THAT: (1) CANCELLATION REQUIRES A MINIMUM OF 31 DAYS' NOTICE: (2) ONE FINAL REGULAR MEMBERSHIP PAYMENT WILL BE DUE AFTER I SUBMIT MY CANCELLATION REQUEST: AND (3) IF I CANCEL MY MEMBERSHIP WHILE IT IS FROZEN WITHOUT FIRST REACTIVATING IT, ALL FREEZE FEES PAID AND ANY CORRESPONDING ACCOUNT CREDIT WILL BE FORFEITED.
